Together in health

This community based wellness program facilitates wellness education and screening through coordination with churches and civic organizations. The focus is to promote wellness screenings and educate Laurens County citizens about healthy lifestyles and disease management.

The Congregational Nurse Program is a new vision for health. To participate in the program, a congregational nurse or congregational healthcare promoter (CHP) joins the pastoral team to coordinate a whole person health ministry. The mission of the Together in Health Program is for the congregational nurse or CHP to provide physical, emotional and spiritual care for individuals, families and communities served by local churches, in association with health care resources. The vision is to develop collaborative partnerships between agencies of healing, churches and hospitals that will make a fundamental difference in the health status of communities that are served. The congregational nurse or CHP facilitates the use of resources within the congregation and community to provide services based on the assessed health needs of that particular congregation. The congregational nurse or CHP does not duplicate existing health services, but seeks to bridge the gaps by delivering and coordinating educational, preventative and therapeutic services not otherwise available.
